In consideration of my child or ward ("Participant") being permitted to take part in training sessions, camps, and related activities offered by Ballogic Soccer Training ("Ballogic"), I, the undersigned parent or legal guardian, acknowledge and agree to the following:
1. Assumption of risk. I understand that participation in soccer training and athletic activity involves inherent risks, including physical injury, illness, and in rare cases serious injury. I knowingly and voluntarily assume all such risks on behalf of myself and the Participant.
2. Release of liability. I release, waive, and discharge Ballogic, its owners, coaches, employees, and volunteers from any and all claims, liabilities, or damages arising out of the Participant's participation, except those resulting from gross negligence or willful misconduct.
3. Medical authorization. I authorize Ballogic staff to seek emergency medical treatment for the Participant if needed, and I am responsible for any related costs. I confirm the Participant is physically able to participate or has been cleared by a physician.
4. Photo and media release. I grant Ballogic permission to use photos or video of the Participant from activities for promotional purposes. I may opt out by emailing info@ballogic.com.
5. Agreement. I have read and understood this waiver. I am the parent or legal guardian of the Participant and agree to its terms on their behalf. I understand this is a binding agreement.
